Cam gears are a mod for an E34 M5 yes. Is the E34 M5 motor the same as the E36 M3 motor? No only no, but hell no.

The E34 M5 motor is basically the same as the E28 M5 and E24 M6 motor (and further back the M1). It is an S38 B36 (or B38 but not in the US). The E30 M3 motor is an S14 and is basically an S38 minus 2 cylinders.

The US E36 M3 motor is the S50US B30 and S52 B32 (not to be confused with the Euro only S50 B30 and S50 B32). The US M3 motors have vanos (variable valve timing) on the intae cam- something that is not present on the E34 M5 (or any of the motors discussed in the above paragraph).

Cam gears changes are not needed on a car with variable valve timing. That is what the variable valve timing is for. If you do change cam gears you better have the correct tools to retime the Vanos.
