Car/Key Programming Request

Does anybody have a current copy of a complete Car & Key Memory form that they received from their dealer for programming their 2007 X3's key and alarm functions?

I have a copy of the one page BMW form I found on another website (this one is quite incomplete.)

I also have the one that is posted on X3world which is a year old - but I would really like one that somebody has that they actually recieved from their dealer!

I have the same question for my 07. The list I saw showed about 3 key items that were programable, but when you look at the owners manual there might be alot more.
