I didn't have any issue feeling it back in blindly, I think you'll be surprised. Worst case scenario use the dental mirror and a flashlight to get a visual once its back up there.

No worries, it's been American proofed so you shouldn't ever have a problem. No other nozzle besides a diesel will even go in the filler neck.

Looks like perhaps they don't use a TPS (throttle position sensor). Those may have actually gone away with electronic throttle controls, or at least perhaps they are integrated into the same unit...

I lost a throttle position sensor on my '00 silverado, it was a cheap part and easy to replace. I'd start there if your are somewhat handy with a wrench.

I doubt the fuel cap had anything to do...
