It is designed that way to avoid blinding others i believe. Mine also does not moves to the opposite traffic side AFAIK. Havent paid attention to it since i dont drive the E60 as much as before.

I'd say that you should change it once a year since you don't really drive that much.
That's what i do with mine since i only drive around 9k kilometers per year which is around 5.6k miles a year....
