Not necessarily. Once the pulley wheel gets loose it can make a few different types of sound, including what you're describing. The pulley wheel itself can be replaced for cheap and not even that...

Agree, check the part diagrams. That's on your fuel injector rail, and looks to be a pressure regulator of some sort if I had to guess..... don't know the 4-bangers as well as the I-6.
