Change all 4 while you are back there in that tight space. I did all of mine when I did the valley pan and TCGs. I didn't want to face a busted hose down the road.

Just curious if it is the M62TU engine? If yes, then the answer to your question is no on the cylinder head removal. I would drop the lower oil pan first and look for the telltale plastic guide...

Leaks at the back of the engine could be from one of the four heater hoses back there. (They are rubber). Another possibility is a leak from the valley pan. You can use search here to find more...
