E3 O2 sensor adaptation limit Cyl #1-3
E4 O2 sensor adaptation limit Cyl #4-6

I cleared them and drove to work about 12 miles with no codes.

With only 54k is it likely the O2 sensors are failing or that I have a vacuum leak or could it be something else?

The pre cat O2 sensors seem to be the only ones that fail compared to the rears, Your mileage is pretty low for that but O2 senors are working as long as the engine is running so if the car is always in traffic it may have low miles but higher use on the o2's
