Mine occurred at 16,000 miles.
12,000 mile service was done and DEF low indicator was never triggered
Dealer topped off no charge (Both tanks were almost empty and took a little over an hour to fill)
Dealer checked out system and it was working fine
* My feeling is that the system was not refilled at 12,000 miles but I have no evidence to back that up
