Heater/AC Blower malfunction

Months ago I noticed heater makes slight cricket like chirp when fan set on low setting. Past experiences told me I might be having early symptoms of blower motor seizing issue. I would then turn it up higher, chirp would stop. Couple weeks ago when temps were 20 below zero, blower motor made chirp sound (faint level) at all fan speeds. This morning as I was taking kids to ski hill blower motor wasnt coming on at all. Control unit seems to be working fine. Checked all fuses and good there. While driving I put my hand above defrost vent and could feel warmth but no forced air. Tried pulling #50 fuse to reset FSR switch...no luck. A few minutes ago I started car and heater blower came on for a few seconds and then stopped. I hope its FSR and not blower. What do you guys/gals think???
