limp mode and no boost

Hi can anyone help with this one?
My E53 3.0 diesel has acceleration problems-when you need power to go fast or up hills it seems to not have the boost anymore,and revs go up but car slows down to a snails pace!! Changed MAF,had its MAF codes reset,still same any ideas???
