4.6is black smoke and hesitation help!

Wife was driving the 2003 4.6is today drove the car for 20 min on the highway (outside temp was -15c) . A hesitation was felt while accelerating, followed by a large, thick black cloud of smoke from exhaust (could not see out of rear window)...lasted maybe 20-30sec. The smoke stopped for another minute or so, then it happened a second time. No warning lights or other issues noticed at this time. Exited the highway, continued to feel hesitations, but no more smoke. At stop light, could ?smell burning?. Pulled over and check engine soon light stared flashing. turned off engine. Since it so cold out won't be able to take a look at the truck until the weekend so i am trying to get some ideas as what to look for? The smoke was defiantly black so I am thinking it may be fuel related? or could it be ccv issue? Any thoughts would be helpful.

Fuel is not burning properly. Could be anything from a bad fuel pump, bad ignition, or bad fuel. Has it been sitting for a while? Can you get it to an OBDII ?
