Pry out the mirror control switch, there is a torx screw in there. behind the interior door handle, there will be one more screw. Look carefully for that one as it has a plastic cap covering it. Once you have the two screws removed you can begin pulling the door panel out from the door. Begin at a bottom corner and work your way across then up each side. It is held on by plastic snaps. Once you have all the snaps unfastened, lay on your back under the door with a flashlight. behind the door panel just about center is a clip that you need to release with a screw driver. You will see what I mean when you get to that point.

After all that is done, unscrew the door lock knob and wiggle the panel upward and out. A few of your plastic snaps will break but they are about a buck each from the dealer or can be ordered on line.

Or LOL... (Just found this after I typed all the crap above.)

After reading your comment, I couldn't help but sign up so I could say thanks. :-)

My driver's side window just fell forwards in its guide, so I'll be going in the door panel tonight to fix it. I'm assuming that the forward guide nut fell off because I heard what sounded like a quarter falling through a gumball machine.

I think because it has a title that a lot of people search for (in Google or Yahoo or whatever kids are using nowadays), and it also contains a link to e34.net with a detailed DIY of it. I can't imagine how many views his site gets in a day. I know I've used it at least 20 or 30 times.
