Yes, #3 holds the caliper bracket to the wheel. If you aren't changing the rotor, you don't need to remove the #3 bolt.

I have a 2006 530xi. I did the replacement through the passenger side wheel well. Removed some of the plastic wheel well shields and that was enough to get it done.

If it is intermittent, it could well be a bad connection. When it isn't working, check the heater coil resistance. If it is open, something is wrong in that segment. Divide and conquer.
