This was odd and I'm surprised it took me a long time to find it. I had some oil on top of my valve cover and finally while wiping it up I noticed that the washer under one of the valve cover bolts was spinning and realized it was missing the rubber grommet underneath it. It was on the top, first one under the plastic cover and could be partially seen with the cover on.

Anyone ever have this happen before? Seems odd that the grommet would be completely gone.

Crumbled and fell into the engine? Hopefully not ... had any work done to the top of the engine?

The PCV can't work if there's a big leak -- getting any codes?

No idea what might have happened to it. There was no work done to it since I've owned it that would require the valve cover bolts to be removed. Valve cover gasket was replaced over 2 years ago I think.

Had some codes for misfires before I changed the spark plugs.

Wasn't losing a huge amount of oil. About 600 ml in 3,000 miles.
