I drove from Philly to Washington this weekend.... I was doing between 70-90mph the whole time and the car never went past 4th gear. I did this intentionally (it's a manual). I found that of course the acceleration was much more on tap, but even better - my miles per gallon jumped UP 2mpg!!! The car was around 4-4.5k most of the time.

Is this a "duh"? Or are there any negative effects from not using 5th gear?

No real reason to. This is why I really want a lower diff in my car. It's not like I'll miss anything off the top end.

Maybe I'm just crazy, but I've noticed that when I drive a little more "spiritedly", I've gotten better fuel economy. But that may because I'm not slowing down as much, hence, no need for more gas ;)
